PIVOTPOINT ADVISORS FORMS
Account Application
This form opens an account with PivotPoint Advisors, LLC and provides us with key information including investment objectives.
Investment Advisory Agreement
This form reflects the contract between you and PivotPoint Advisors, LLC.
FIDELITY ACCOUNT FORMS
Here are the most common forms required to establish an account with Fidelity. If you already have an account at Fidelity, you must still complete these forms to transfer an existing "retail" account to an "advisor" account.
Fidelity Individual/Joint Brokerage Application
This form opens an account with Fidelity Investments and provides PivotPoint Advisors, LLC with the authority to manage your account and deduct management fees.
Fidelity Business/Partnership Account Application
This form opens a business or partnership account with Fidelity Investments and provides PivotPoint Advisors, LLC with the authority to manage your account and deduct management fees.
Fidelity IRA Account Application
This form opens a qualified IRA retirement account with Fidelity Investments and provides PivotPoint Advisors, LLC with the authority to manage your account and deduct management fees.
Fidelity Cash Management Request Form
This form is required if you would like to be able to withdraw funds from your Fidelity brokerage account, via either check or Electronic Fund Transfers.
Fidelity Transfer of Assets Form
This form is used to transfer your cash and/or securities from another brokerage account to your Fidelity account. The form is also used to transfer an existing "retail" account to an "advisor" account.
